The process

What happens after a death in Poland

Contact emergency services (112). Death must be registered with the local USC (Urzad Stanu Cywilnego) within 3 days. The Prokuratura (Prosecutor's Office) takes jurisdiction for violent or suspicious deaths. The akt zgonu is issued in Polish and requires certified English translation for UK and Irish destinations. Poland has been a Hague Apostille Convention member since 1992.
