Cook County Medical Examiner
Deaths in Chicago and Cook County fall under the jurisdiction of the Cook County Medical Examiner’s Office, based on West Harrison Street. The Medical Examiner must certify any death that is sudden, unexpected, violent, or where the cause is unknown. This is a mandatory step before a death certificate can be issued.
For natural deaths where a physician can certify the cause, the Illinois death certificate process is faster — typically 3 to 5 working days. Where the Medical Examiner is involved, allow 1 to 3 weeks. Autopsies ordered by the Medical Examiner can extend this further.
Certified copies of Illinois death certificates are obtained through the Illinois Department of Public Health (IDPH) or the Cook County Clerk’s Office.

O’Hare International Airport cargo
O’Hare is a major international cargo hub. Human remains for international air shipment leave through the cargo facilities on the west side of the airport. British Airways operates direct ORD-LHR flights. The licensed funeral home handling the case coordinates the cargo booking and transfers.
Documentation required for export from Chicago:
- Illinois death certificate (certified copy)
- Embalming certificate
- Freedom from infection certificate
- Transit permit issued by the Cook County Clerk
- Funeral home transit authorisation
- Passport of the deceased
Illinois requires embalming for any body transported more than 150 miles or across state lines, which effectively makes embalming mandatory for all international repatriations from Chicago.
