The shared language creates a misleading sense of simplicity. Families dealing with a death in America often assume the process will be more straightforward than somewhere like Thailand or India. In reality, the United States is one of the most complex and expensive countries for repatriation – not because of bureaucratic opacity, but because there is no single US system. There are 50 state systems, each with its own laws, its own vital records office, and its own approach to investigating sudden deaths.
A death in Florida and a death in New York follow different procedures. The death certificate will be issued by a different state agency, at a different speed, with different certified copy costs. Some states process applications in three days. Others take two weeks.
Who investigates the death
The US uses two different systems for investigating sudden or unexplained deaths, and the distinction matters. In states with a medical examiner system, the medical examiner is a qualified forensic pathologist. In states with a coroner system, the coroner is an elected official – and in many counties, they are not required to hold any medical qualification whatsoever. The coroner may be a farmer, a funeral director, or a former sheriff.
This does not mean investigations are conducted poorly. But it is worth knowing that the person with legal authority over your loved one’s remains may not be a doctor. If you believe the cause of death was incorrectly determined, that distinction matters for any follow-up steps you take.
The hospital bill problem
This is the part of a US death that genuinely surprises families. American medical costs are high – not moderately high, but potentially hundreds of thousands of dollars for intensive care, surgery, or trauma treatment. Those bills are generated before death, during treatment.
In some US states, a hospital or healthcare provider can place what is effectively a legal claim against the estate of the deceased – and in a small number of cases, this can create complications in releasing remains until the billing situation is addressed. This does not happen everywhere, and it is not the standard experience. But it is a known risk, and families should be aware of it when travel insurance is absent or inadequate.
The FCDO recommends travel insurance for the United States with at least USD 1 million in medical cover. Standard UK travel insurance often caps at GBP 5 million but imposes sub-limits on treatment in the US. Check your policy before travelling.
The death certificate shows cause of death
Unlike Spain, France, Thailand, or most other countries, US death certificates state the cause of death clearly. This is useful for UK probate and insurance claims. It can also mean that families learn how someone died from a document rather than from a conversation – something to be aware of when handling paperwork.
US death certificates may require an apostille for international legal use, depending on what the UK institution handling the estate requires.
Distance and the cost of air freight
Air freight from the US to the UK is the dominant cost in any repatriation. Flights from New York or Boston run seven to eight hours; from Los Angeles or Las Vegas, ten hours or more. Cargo rates reflect both the distance and the considerable overhead of the American funeral industry, which operates at rates significantly higher than European equivalents.
If the death occurred in a state without a direct cargo route to the UK – parts of the Midwest, the Mountain West, the Deep South – there may be an internal transport leg to reach a major hub airport. Cross-country road transport in a refrigerated hearse is not uncommon for deaths far from New York, Los Angeles, Chicago, or Miami. That adds cost and time.
The total range for a US repatriation is typically GBP 5,000 to GBP 15,000. It is not the most expensive long-haul destination, but it is consistently at the upper end.

First things first
What to do in the first 24 hours
The immediate period after a death abroad is disorienting. Here are the steps in the order they normally need to happen.
Contact local emergency services
Call 911 for emergency services. If death occurs in a hospital, the hospital handles initial procedures. If death is unexpected, the medical examiner or coroner will be involved (this varies by state and county). Contact the British Embassy in Washington DC or the nearest consulate.
Local emergency number: 911
Contact the British Embassy or consulate
Notify the British Embassy in Washington DC as soon as possible. They can give you a list of local English-speaking funeral directors and explain what the local authorities will need.

Appoint a local funeral director
A local funeral director in United States will take care of the body, arrange embalming, obtain the necessary documents, and coordinate with airlines. The embassy can recommend accredited directors. You can also contact a specialist UK repatriation company, who will coordinate with a local partner on your behalf.
Contact your travel insurer
If your loved one had travel insurance with repatriation cover, contact the insurer immediately. They will often have an emergency assistance line and may appoint their own funeral director. They may cover the full cost of repatriation, which can be GBP 5,000-15,000.
Travel insurance with medical and repatriation cover is absolutely essential for the USA. Standard travel insurance may have inadequate medical limits for US healthcare costs. Policies should have at least USD 1 million medical cover for the USA.
Gather the required documents
Repatriation from United States requires specific paperwork before a body can be transported. Your local funeral director will handle most of this.
- US death certificate (state-issued, certified copy)
- Embalming certificate
- Transit permit (issued by state or county)
- Consular mortuary certificate (if applicable)
- Passport of deceased (or certified copy)
- Apostille on death certificate (if required by UK authorities)
- Airline cargo documentation
Documentation typically takes 5-14 days (varies significantly by state) to complete.

What to expect
How long does it take?
Factors that can extend the timeline
- State-by-state variation in processes and timelines
- Medical examiner investigation (holds body until released)
- Death in a state without direct UK flights (requires internal transport)
- Hospital bills must be addressed (hospital may place lien on remains in some states)
- Apostille requirements for death certificate
- Weekend/holiday closures at state vital records offices
- Cross-state transport complications
- Criminal investigation (particularly for violent deaths)
Cost guide
How much does it cost?
| Local funeral director | GBP 1,500-4,000 |
| Embalming | GBP 600-2,000 |
| Documentation | GBP 200-600 |
| UK reception | GBP 500-1,000 |
The USA is one of the most expensive repatriation destinations. US funeral industry costs are significantly higher than European equivalents. New York City, San Francisco, and Los Angeles are the most expensive areas. Florida is mid-range. Rural states may be cheaper but lack direct UK flights, adding internal transport costs.
If a post-mortem is required
Autopsy ordered by medical examiner or coroner if cause of death is unknown, suspicious, or involves violence. System varies by state (some states use medical examiners, others use elected coroners).. Can delay repatriation by 1-4 weeks. Medical examiner holds jurisdiction over the body until released.
Post-mortems in United States are conducted by Medical examiner (qualified pathologist) or county coroner (may be elected, not necessarily a doctor). System varies by state and county..
Common questions
Questions families ask about deaths in United States
Repatriation from United States typically takes 10-21 days. The fastest is 7-10 days with no complications. Complex cases involving a post-mortem or police investigation can take 3-8 weeks.
The typical cost is GBP 5,000-15,000. This covers local funeral director fees, embalming, a zinc-lined coffin, documentation, air freight to the UK, and reception at a UK funeral home. The main variable is air freight, which depends on the destination airport and flight frequency.
Your local funeral director in United States will gather most documents on your behalf. The core documents required are: a local death certificate, an embalming certificate, a freedom from infection certificate, and airline cargo documentation. The full documentation process typically takes 5-14 days (varies significantly by state).
Cremation in United States is available. If your loved one is cremated abroad, returning ashes to the UK typically costs GBP 500-1,500 (flight ticket to carry personally). GBP 300-800 if shipping.. Do NOT cremate abroad if a UK coroner may need to hold an inquest. Cremation destroys evidence.
